A Community Health Worker is a patient advocate in a community setting. Their role is to help the patient access community services specific to their needs, work as a medical provider, and advise patients on issues related to breast cancer screening, pep smears, glaucoma tests, or diabetes screening. Their responsibilities are to identify the particular health care needs of a patient, maintain updated patient records with plans, advise patients about hygiene or sanitation, attend community meetings or health fairs to understand community issues, conducts home visit for pregnant women, newborn children or other high-risk individuals, and contact patients in person, by phone or in writing.
